Care teams are accountable for what happens to patients between visits, yet that's exactly where they have the least visibility. We built Vitality Call to close that gap.
Every clinician knows the pattern. A patient stops taking a medication in week two, mentions it to no one, and the consequences show up months later as an avoidable visit, or worse. The information existed the whole time. There was simply no one whose job it was to ask, every single day, and write down the answer.
Staffing that job with people doesn't scale. Solving it with apps and devices asks the most vulnerable patients to adopt technology they don't want. A phone call, though, is something every patient already answers, especially when the voice on the other end is warm, remembers them, and shows up every day without fail.
So we built Sarah: an AI care manager that talks with every enrolled patient daily and delivers what they report, structured and routed, to the team accountable for their care.
We are deliberately modest about what this product is. It does not diagnose, it does not make clinical decisions, and we will not publish outcome claims we haven't earned. It reaches patients, collects what they report with the consistency of validated screening questions, and puts that information in front of the right people. We think that alone is one of the most underrated levers in healthcare.
